EPCR (PROCR/CD201/activated protein C receptor) is a type I transmembrane N-glycosylated glycoprotein structurally related to the MHC class I/CD1 superfamily, featuring an extracellular domain with two α-helices and eight β-strands forming a hydrophobic lipid-binding groove. It is highly expressed on endothelial cells of arteries and veins in heart and lung, as well as on various stem/progenitor cells. Functionally, EPCR enhances protein C activation to APC by the thrombin-thrombomodulin complex, regulating coagulation; it also mediates cytoprotective signaling via PAR1, promoting cell proliferation, angiogenesis, and anti-inflammatory responses. A soluble form (sEPCR) is released by metalloproteases such as ADAM17. Clinically, EPCR represents a promising therapeutic target—its inhibition attenuates pathological retinal neovascularization, while APC-EPCR axis activation shows efficacy in wound healing, sepsis, and retinal protection, with non-anticoagulant APC variants being developed to minimize bleeding risks.
