Leptin is a 16-kDa adipokine, a hormone primarily secreted by white adipose tissue. Its discovery revolutionized the understanding of energy homeostasis, establishing fat tissue as an active endocrine organ. Leptin's primary biological role is to communicate long-term energy sufficiency to the brain. It circulates at levels proportional to body fat mass and acts by binding to specific leptin receptors (Ob-R) in the hypothalamus. This binding activates the JAK-STAT signaling pathway, ultimately suppressing appetite and increasing energy expenditure while also modulating neuroendocrine functions, immune responses, and reproduction. Consequently, leptin serves as a critical anti-obesity signal, promoting satiety and metabolic balance. However, in most common forms of obesity, high circulating leptin levels fail to suppress appetite, indicating a state of leptin resistance, a central pathophysiological feature of metabolic syndrome. The significance of leptin detection is multifaceted in both clinical and research settings. Quantifying serum leptin levels, typically via ELISA, provides a direct biomarker of adipose tissue mass. Clinically, it is crucial for diagnosing rare congenital leptin deficiency, a condition treatable with recombinant leptin. More broadly, measuring leptin aids in researching the mechanisms of leptin resistance, metabolic dysregulation, and the links between obesity, type 2 diabetes, and cardiovascular diseases. In nutritional studies, leptin levels help assess energy status and metabolic adaptation. Furthermore, as leptin influences immune and inflammatory pathways, its detection is relevant in studies of autoimmunity and cancer. Therefore, leptin assays are vital tools for diagnostic evaluation, understanding pathophysiology, and monitoring metabolic health.
