Glypican-3 (GPC3) is a heparan sulfate proteoglycan that plays a significant role in cell growth, differentiation, and migration. It is a member of the glypican family and is anchored to the cell membrane through a glycosylphosphatidylinositol (GPI) anchor. GPC3 is highly expressed in fetal tissues such as the liver and placenta but is typically absent in healthy adult tissues, except for minimal expression in the heart, lungs, kidneys, and ovaries. However, it is notably overexpressed in various cancers, including hepatocellular carcinoma (HCC), melanoma, and some pediatric cancers. This differential expression pattern makes GPC3 a promising target for cancer diagnosis and therapy. In HCC, GPC3 acts as an oncofetal protein, reactivating the fetal phenotype often seen in malignant tumors. It modulates cell behavior by interacting with growth factors such as Wnt, fibroblast growth factor (FGF), and insulin-like growth factor, influencing cell proliferation, adhesion, and migration. Due to its high specificity in cancerous tissues, GPC3 is being explored as a target for immunotherapies, including vaccines, immunotoxins, and chimeric-antigen-receptor T-cell therapies. Additionally, its presence in the blood of cancer patients makes it a potential biomarker for liquid biopsies, aiding in the detection and monitoring of disease progression.
