TDP‑43 (TAR DNA‑binding protein 43) is a pivotal pathological biomarker originally well‑recognized for diagnosing TDP‑43 proteinopathies, including amyotrophic lateral sclerosis (ALS) and frontotemporal lobar degeneration (FTLD‑TDP). Pathologically modified TDP‑43 undergoes hyper‑phosphorylation, truncation and cytoplasmic aggregation in neurons, triggering neuronal damage and disease progression, often preceding overt clinical symptoms.
Beyond ALS and FTLD, emerging evidence highlights its critical diagnostic and prognostic value in Alzheimer’s disease (AD). TDP‑43 inclusions are detected in over half of AD patients, typically with limbic distribution, and correlate with exacerbated cognitive decline independently of classic AD pathologies like amyloid‑β plaques and tau tangles. TDP‑43 testing helps differentiate pure AD from AD mixed with TDP‑43 pathology and limbic‑predominant age‑related TDP‑43 encephalopathy (LATE), resolving clinical diagnostic confusion caused by overlapping dementia phenotypes.
Biofluid TDP43 quantification improves diagnostic accuracy, enables disease subtyping, and predicts functional deterioration in neurodegenerative disorders. It also serves as a key pharmacodynamic biomarker for targeted therapy trials. Despite ongoing assay standardization efforts, TDP43 detection facilitates early intervention, precise patient stratification and personalized prognosis across ALS, FTLD and AD spectrum diseases.
