Complement Factor B (CFB) is a key serine protease precursor that exclusively activates the alternative complement pathway, serving as an essential regulator of innate immune defense and inflammatory homeostasis. Synthesized mainly by the liver and monocytes, CFB is cleaved into Ba and Bb fragments to form stable C3 convertase, driving complement amplification, pathogen clearance, and immune surveillance. Dysregulated CFB expression and activation directly trigger excessive complement cascade activation, leading to tissue inflammation and microvascular injury.
Serum CFB detection provides specific laboratory evidence for distinguishing alternative pathway activation from classical complement pathway disorders. Elevated CFB levels reflect active inflammatory responses, commonly observed in autoimmune diseases, glomerulonephritis, and systemic infections. In contrast, reduced CFB concentrations indicate impaired complement synthesis due to severe liver dysfunction or excessive complement consumption in acute immune-mediated injuries such as atypical hemolytic uremic syndrome.
Clinically, CFB quantification assists disease subtyping, aids differential diagnosis of complement-related microangiopathies, and monitors inflammatory disease activity. Serial CFB measurement enables dynamic evaluation of disease progression and treatment response. As a specific biomarker of alternative complement pathway activation, CFB testing improves the precision of immune-mediated disease diagnosis and supports individualized clinical management.
