Leukemia inhibitory factor (LIF), a pleiotropic member of the IL‑6 cytokine family, holds great diagnostic and prognostic value for inflammatory conditions, especially cytokine storm. Produced by macrophages, endothelial and stromal cells, LIF shares the gp130 receptor subunit with IL‑6 and activates the JAK‑STAT3 inflammatory cascade. Beyond its original function regulating leukemia cell differentiation, circulating LIF serves as a promising soluble biomarker for severe systemic inflammation.
During cytokine storm triggered by CAR‑T therapy, sepsis or acute tissue injury, serum LIF rises synchronously with IL‑6, TNF‑α and acute‑phase reactants. Its concentration positively correlates with inflammatory magnitude and end‑organ damage, reflecting hyper‑activated immune loops that amplify pro‑inflammatory mediator release. Measuring LIF complements conventional cytokine panels: it helps stratify storm severity, monitor dynamic inflammatory progression, and identify high‑risk patients before irreversible organ failure occurs. LIF testing also supports therapeutic evaluation for anti‑inflammatory interventions. Though dual pro‑ and anti‑inflammatory properties exist, serial LIF detection provides extra clinical insight to refine risk assessment for life‑threatening cytokine‑driven syndromes.
